Varicocele: surgery or natural treatment

What is varicocele?

Varicocele is one of the most common pathologies of the male genital system: it is an abnormal dilation of the testicular veins which, becoming incontinent, causes pathological reflux of venous blood from the abdomen to the testicle, causing stagnation of the blood, increased scrotal temperature, poor oxygenation and an accumulation of potentially harmful substances. Generally, the disease appears between 15 and 25 years of age and, in most cases, occurs between 11 and 16 years of age, during the pubertal development phase.

Symptoms

Varicocele, very often, is completely asymptomatic. However, in cases where he has symptoms, it is: pain in one or both testicles, feeling of heaviness or discomfort in the scrotum (especially after a day spent standing or after practicing a sport), palpability of varicose veins in the scrotum, hypotrophy of the testicle.

These depend on the stage reached by the pathology. Usually, the diagnosis is made by the doctor through a precise inspection of the scrotum, which identifies the presence of spider veins in the testis. In addition, as varicocele can compromise the quality of the seminal fluid, certain tests are necessary for a precise evaluation: in particular, it is the spermiogram and echo-color testicular Doppler (essential even after the treatment to evaluate its success, a possible persistence of venous reflux or a possible recurrence).

The staging involves four grades:

• subclinical stage: not obvious on palpation or observation, but detectable only during instrumental examinations;

• varicocele of the 1st degree: detectable by palpation, through the so-called "Valsalva maneuver";

The Valsalva maneuver is a manual maneuver that is performed by exhaling after a deep breath, keeping your mouth and nose closed. More scientifically, we can say that it is a maneuver characterized by:

-a deep inspiration followed

- forced exhalation with closed glottis

-for approximately 10 seconds.

This maneuver was well known to Arab doctors from 1100 AD but only several centuries later, around the XVI-XVII AD, it was introduced into clinical practice in Italy thanks to the scientist Antonio Maria Valsalva.

• 2nd degree varicocele: detectable by palpation of the testicle;

• 3rd degree varicocele: more advanced stage, clinically detectable both with observation and with palpation.

Varicocele occurs in 95% of cases in the left testicle and is common in men with fertility problems.

 

Intervention

Surgical treatment of varicocele may require several types of surgery. More specifically, you can use:

• retrograde scleroembolization, a recent, minimally invasive procedure, which uses specific and sophisticated catheters;

• ligation of the spermatic veins, traditional intervention which can be carried out at different levels (sub-terminal, inguinal, retroperitoneal);

• sub-terminal microsurgical scleroembolization;

• retrograde transfemoral scleroembolization (performed in interventional radiology).

The operation for varicocele, in general, is successful in 90% of cases and, generally, there are no serious complications. During the postoperative course, the testicle may remain painful for a few weeks.

 

Medication

The most commonly used varicocele medications are based on:

• Sodium tetradecyl sulfate 3%;

• Ethanolamine oleate;

• Laureth-9 (polidocanol).

The dosage depends on the severity of the varicocele and the condition of the patient.

Non-surgical remedies

The main objective of treatment for varicocele is to improve spermatogenesis or prevent its future deterioration. If the need for surgical treatment is not recognized, it is however advisable to carry out periodic andrological checks. Prevention of varicocele is not possible, but, in the presence of pathology, useful rules of behavior can be adopted such as:

• use of briefs and pants that are not too tight, in order to avoid increasing the temperature and rubbing;

• avoid standing for a long time, especially in hot environments;

• use of jockstrap when practicing sports activities.

It is also advisable to follow a diet based on fresh fruits and vegetables (rich in flavonoids), whole grains, fish, essential fatty acids, nuts and extra virgin olive oil. It is also necessary to drink lots of water and avoid the consumption of animal fats, margarine, hydrogenated oils, sugar, white flour products, caffeine and alcohol.

In certain case varicocele doesn't need to be treated surgically if it is not well developed, if it doesn't cause discomfort and infertility.
